Please have the following ready:
- Photo ID (passport or license)
- Insurance card if applicable picture of front and back
- A credit card for payment.
- NOTE – Telemedicine visit insurance copays range from $20-$60. For services not covered via insurance or patients w/o insurance cost: $89 for established patients and $99 for new patients excluding labs.

| Chest Pain | Trouble swallowing/breathing |
| Shortness of Breath (moderate or severe) | Medical Emergencies |
| Prescription controlled substances | Poisoning or overdose |
| Severe pain | Moderate or severe abdominal pain |
